How the Process Works
Purchase: Complete your payment online.
Required Information Call: After purchase, a representative will call you to collect additional personal information required to finalize your Quest Diagnostics order.
Receive Your Form: Once we have your information, your official screening form will be emailed to you.
Visit a Lab: Take your form to a participating Quest Diagnostics facility.

Important Policies & Validity
The 180-Day Rule: Per USCG regulations, your drug screen must be dated within 180 days of your application submission.
Expiration & Repurchase: If you do not submit your application to the Coast Guard within this 180-day window, your test is no longer valid. No refunds will be issued, and no new tests will be provided without a full repurchase.
Quest Select Member Locations: This voucher is not valid at "Quest Select Member" locations.
If you use a Quest Select site, additional fees will be charged to Explorers Guide.
You must reimburse Explorers Guide for these fees before your test results will be released to you.
Refund Policy: All sales are final. Drug screen payments are strictly non-refundable.
